School Physicals: Required for K-12 students in Florida. We complete all necessary forms during your visit, including Florida Department of Health Form DH 3040.
Sports Physicals: Complete sports participation evaluations for student athletes. We assess cardiovascular health, musculoskeletal function, and overall fitness for sports.
Both are covered services under Florida Medicaid. Schedule your physical early to avoid the back-to-school rush in July and August. We can often accommodate same-day appointments for physicals.
